Chronic lung disease after premature birth.

In 1967, northway et al. first described a new chronic respiratory disease, bronchopulmonary dysplasia, that developed in premature infants exposed to mechanical ventilation and oxygen supplementation.1 Two decades later, the same authors found that clinically significant respiratory symptoms and functional abnormalities persisted into adolescence and early adulthood in a cohort of survivors of bronchopulmonary dysplasia,2 suggesting that lung injuries early in life may have lifelong consequences. Bronchopulmonary dysplasia is now the most common chronic lung disease of infancy in the United States. Today, newborns consistently survive at gestational ages of 23 to 26 weeks — 8 to 10 weeks younger than the infants in whom bronchopulmonary dysplasia was first described. New mechanisms of lung injury have emerged, and the clinical and pathological characteristics of pulmonary involvement have changed profoundly, although its natural history and outcome into adulthood are still largely unknown. It is only now that large populations of persons born prematurely are approaching adulthood, and they may be at increased risk for respiratory disease in adult life.

Unstudied infants: outcomes of moderately premature infants in the neonatal intensive care unit.
BACKGROUND Newborns of 30-34 weeks gestation comprise 3.9% of all live births in the United States and 32% of all premature infants. They have been studied much less than very low birthweight infants. OBJECTIVE To measure in-hospital outcomes and readmission within three months of discharge of moderately premature infants. DESIGN Prospective cohort study including retrospective chart review...
متن کاملPaediatric origins of adult lung disease
Bronchopulmonary dysplasia (BPD) is the most common form of chronic lung disease in infancy. The clinical, radiological, and pathological features of BPD were first described a little more than three decades ago. The disease was then seen in large preterm infants with severe respiratory distress syndrome who had been treated with high inspired oxygen concentrations and prolonged mechanical vent...
متن کاملImmature lung and acute lung injury.
Acute lung injury occurs mostly in the very low birth weight and extremely low birth weight infants. The pathological process leading to acute lung injury includes immature and/or diseased lung that experienced oxidative stress, inflammation and mechanical insult with the bronchial, alveolar and capillary injuries and cell death. It may be the first step to the subsequent development of chronic...
متن کاملAberrant Pulmonary Vascular Growth and Remodeling in Bronchopulmonary Dysplasia
In contrast to many other organs, a significant portion of lung development occurs after birth during alveolarization, thus rendering the lung highly susceptible to injuries that may disrupt this developmental process. Premature birth heightens this susceptibility, with many premature infants developing the chronic lung disease, bronchopulmonary dysplasia (BPD), a disease characterized by arres...
متن کاملThe Use of Bubble CPAP in Premature Infants: Local Experience
Received February 15, 2007 Abstract Bubble continuous positive airway pressure (CPAP) was introduced more than 30 years ago for infants with respiratory distress. With this, there had been reports of decreased incidence of mechanical ventilation and chronic lung disease among the premature, as well as less failure of extubation. This report described how this treatment modality was adopted loca...
متن کاملPulmonary function changes after nebulised and intravenous frusemide in ventilated premature infants.
AIMS To compare the effects of a single dose of frusemide administered either intravenously or by nebulisation on pulmonary mechanics in premature infants with evolving chronic lung disease. METHODS The effect of frusemide on pulmonary mechanics was studied at a median postnatal age of 23 (range 14-52) days in 19 premature infants at 24 to 30 weeks gestational age, who had been dependent on m...
